In case of a medical emergency, I accept the following terms:
In the event of illness or accident, I give permission for medical treatment to be administered where considered necessary by a suitably qualified medical practitioner and/or hospital.

I understand that every effort will be made to contact me as soon as possible.

In an emergency I can be contacted at the telephone numbers supplied.

I agree to allow the child named above to take part in Ealaín Karate Dojo training and grading.

I understand that there will be suitable supervision while the young people are in the care of the Ealaín Karate Dojo.

I also agree & acknowledge that my photograph/video may be taken during participation in training or events and may be used by Ealaín Karate Dojo on social media, or appear on the Ealaín Karate Dojo website. We will never identify you without your prior agreement.

I agree to abide by the Dojo Kun (rules of the dojo):
1. Seek perfection of character.
2. Be faithful.
3. Endeavour.
4. Respect others.
5. Refrain from violent behaviour.

I understand that if I fail to observe the above Dojo Kun, I may be asked to leave the dojo.
